A completed Gaming Activity Report Request Form may be submitted for processing in one of two ways:


  1. By mail to address to listed on form*
  2. In person at the Boyd Rewards Club

 

*Notary not required if form is requested or presented in person. Please allow 2 – 4 weeks for request processing.
